In consideration of being permitted to participate in any way at Elevate Sports, LLC., I, for myself, my heirs, personal representatives, and assigns, do hereby release, waive, discharge, and covenant not to sue Elevate Sports, LLC., its owners, directors, officers, employees, agents, and volunteers (collectively, the “Released Parties”) from any and all liability for any and all claims, demands, actions, causes of action, or damages of any kind, including but not limited to personal injury, property damage, or wrongful death, arising out of or related to my participation in activities at Elevate Sports, LLC., whether caused by the negligence of the Released Parties or otherwise.
I understand that participation in activities at Elevate Sports, LLC., involves inherent risks, including but not limited to the risk of serious injury or death, and I voluntarily assume all such risks. I agree to follow all safety instructions and to act responsibly to avoid injury to myself and others. I further agree to indemnify and hold harmless the Released Parties from any and all claims, demands, actions, causes of action, or damages of any kind brought by a third party, including but not limited to personal injury or property damage, arising out of or related to my participation in activities at Elevate Sports, LLC.
I have read and fully understand this Waiver of Liability, Assumption of Risk, and Indemnity Agreement, and I voluntarily agree to its terms.
