By agreeing to this waiver and entering the facility, I acknowledge and agree to the following terms as a participant or parent/guardian of a participant using the facilities at Precision Athletics & Wellness, including but not limited to the batting cages, court facilities, and open turf fields.
Safety & Facility Rules: I agree to follow all posted and verbal safety instructions regarding the specific area I am using. I understand that unsafe, reckless behavior, or "horseplay" may result in immediate removal from the facility without refund.
Batting Cages: Helmets must be worn at all times inside the cages. Only one batter is allowed in the cage at a time.
Turf Fields: Proper footwear must be worn; metal cleats are strictly prohibited.
Courts: Proper athletic footwear is required.
Minors: If the participant is under the age of 18, this Agreement must be signed by a parent or legal guardian, who agrees to all terms on behalf of the minor and assumes full responsibility for the minor’s participation. Minors under 18 must be accompanied and supervised by a parent, guardian, or coach at all times while on the premises.
Photo & Video Release: I grant Precision Athletics & Wellness the irrevocable right and permission to take, use, publish, and reproduce photographs and videos of me (or my minor child) for marketing, promotional, social media, advertising, website, and other business purposes, without compensation or notification. I waive any right to inspect or approve the finished product or its use.
Assumption of Risk: I understand that participating in sports and athletic activities involves inherent risks, including but not limited to being struck by balls or bats, slips, falls, collisions with other participants or equipment, and other potential injuries. I voluntarily assume all risks associated with the use of the batting cages, courts, turf areas, and related equipment.
Release of Liability: I hereby release, waive, and hold harmless Precision Athletics & Wellness, its owners, staff, affiliates, and representatives from any and all claims, damages, or injuries that may occur while on the premises or participating in any activity.
Equipment & Property: I acknowledge that I am responsible for any damage to the facility, turf, nets, or equipment resulting from my (or my minor’s) misuse, negligence, or failure to follow facility rules.
I (or I on behalf of my minor/child under the age of 18) confirm that I have read and understand this waiver and agree to the terms and conditions stated above.
